If you're searching for Care Management in Wellesley, you're probably in one of those moments where life has gotten genuinely complicated. An aging parent's care is fragmenting across multiple providers. A family member is navigating a new medical or psychiatric diagnosis. You're holding more medical paperwork, insurance details, and pharmacy logistics than any one person should be expected to track.
Onto Therapy offers Care Management as a standalone service, or as a complement to therapy. Our role is to bring structure, coordination, and clinical perspective to situations that have outgrown a single calendar.
Sandwich-generation adults
If you're balancing your own life with the care of an aging parent — medications, appointments, communication with their doctors, monitoring how they're actually doing — Care Management gives you a partner.
Families navigating mental health or medical complexity
When one family member's care touches multiple providers, systems, and decisions, having a single clinician coordinating reduces both the burden and the risk of things falling through cracks.
Adults managing their own complex care
Sometimes you need a clinical partner who can help with the practical side as well as the personal. We can help with chronic illness, recovery from a major medical event, or managing psychiatric care alongside daily life.
Onto therapy clients with concurrent needs
When a current Onto client is also managing complex care for themselves or someone they love, Care Management can be added to their treatment plan.
Coordination with physicians, psychiatrists, attorneys, and other care providers
Medication management oversight: reviewing prescriptions, tracking changes, flagging concerns
Healthcare navigation: helping you find specialists, schedule appointments, follow up
Family meetings and communication with care teams
Support during life transitions moves, diagnoses, level-of-care changes
Clinical perspective on whether what's happening clinically is what should be happening
Clinical depth, not just logistics
Many Care Management services focus on logistics: scheduling, paperwork, transportation. We bring those things — plus the clinical perspective of a team trained at McLean Hospital, Mass General, and Harvard Medical School. When a question comes up about what's happening clinically, we can actually weigh in.
Hospital-trained leadership
Onto is led by Kerry Rivard, LMHC, former Program Director of the 3-East DBT Residence at McLean Hospital, with two and a half decades of clinical and program leadership experience. Our Scientific Advisor is Douglas Katz, PhD (Mass General, Harvard Medical School).
Integrated with therapy when helpful
Care Management can be a standalone engagement or layered into a broader treatment plan if you or a family member are also doing therapy with us.
Engagements begin with a comprehensive assessment so we understand the situation, the people involved, and what success would look like. From there, we build a Care Management plan suited to your needs — hourly or retainer-based, depending on the scope. You'll know what we're doing, what we're coordinating, and what it costs.
We provide Care Management in person in Wellesley and remotely throughout Massachusetts. Our Care Management clients come from Wellesley, Needham, Newton, Weston, Natick, Dover, Wayland, and across MetroWest. Many are sandwich-generation adults managing parents in Wellesley or surrounding towns while balancing their own demanding lives.
Care Management is private pay, billed hourly or on a retainer basis depending on scope. We accept all major credit cards, FSA, and HSA.
The best way to know if Onto is the right fit is a free 20-minute consultation. We'll discuss your goals, answer your questions, walk through our approach and pricing, and help you decide whether to schedule a full intake.
Other Ways We Help
Care management frequently runs alongside family therapy when complex situations require both clinical and logistical support.
If you're looking for therapy rather than coordination, our broader therapy services cover individuals, teens, and families across the range of presentations.
We also coordinate care for clients receiving specialized treatment such as evidence-based PTSD therapy or structured DBT programs, making sure their providers, medications, and life logistics stay in sync.
